CHECKLIST FOR APPLICATION ENVELOPE
THIS IS VERY IMPORTANT!!
NOTE: Plan ahead for things that may take extra time, i.e.: transcripts, financial aid forms and letters of recommendation.
In order for the application to be processed, you must furnish the following items IN THE SAME ENVELOPE WITH YOUR APPLICATION:
- Updated high school or college transcript including most recently completed semester.
- Two letters of reference from a teacher, employer or clergy (not personal friends or relatives). These MUST accompany your application.
- A copy of the Information Summary of your financial aid form that you have received back from the Federal Government (U. S. Department of Education) or your Student Aid Report (SAR) from the Federal Student Aid Program. You must have the "Information Summary" or SAR you receive back from the Federal Student Aid Program. We will not accept a copy of the financial aid form that you submitted to the college or university you will be attending. If you have not yet received back your summary by the deadline for filing the application (March 1st), you will not qualify for a scholarship this year.
- A one-page (minimum) handwritten explanation of why your circumstances merit scholarship consideration. Your background and personal information are helpful.
Sign and return with your application the whole attached "release of information" form. We will send it to your college or university.
